In order to get quotes, all insurance companies require a census of the Full Time employees and ususally, a medical questionnaire.   Rates are based upon the age, gender, and medical history of employees in the group so this information is most important.    If you have a group policy in place, it is important to provide current rates so we can insist a new carrier     MUST BEAT THESE RATES!!
